Welcome to Magadh Oro Dental’s blog, where we provide insights into dental care procedures. In this post, we’ll explore the types, techniques, indications, benefits and risk of maxillary sinus lift or maxillary sinus augmentation procedure.
A maxillary sinus lift is a dental surgery that increases the bone volume in the upper jaw, specifically in the region of the maxillary sinus. This is done to make it possible to place dental implants in areas where there's not enough bone naturally. The procedure involves lifting the sinus membrane and replacing it with bone graft material, which then helps to regenerate bone over time.
Maxillary sinuses are air-filled cavities in the upper jaw, can expand over time, especially after tooth loss, reducing the amount of bone available for implant placement. Once a tooth has been removed from the back of the upper jaw, the bone height shrinks, which can result in a sinus bone graft being required before an implant can be placed. Indications of Sinus Lift or Augmentation are -
When teeth are lost, the bone that supported them can gradually thin and shrink.
The maxillary sinus, which sits above the upper molars, can expand into the bone if teeth are lost, further reducing the bone available for implants.
If there's not enough bone height or width to support a dental implant, a sinus lift can create the necessary volume for successful implant placement.
Allows for dental implant placement in areas where it was previously not possible.
Regenerates lost bone and increases the bone volume for implant support.
Improves the function and aesthetics of the smile.
As with any surgery, there's a risk of infection.
There's a potential for nerve damage, though rare, as nerves are located in the sinus region.
The sinus membrane can be inadvertently pierced during the procedure.
Open and closed sinus lifting procedures are reliable approaches for increasing the bone volume needed to support proper implant positioning.
A more conservative approach where bone is augmented from the crest of the alveolar ridge. A crestal sinus lift, also known as a transcrestal sinus augmentation, is a minimally invasive dental procedure used to increase bone volume in the upper jaw, particularly in the posterior maxilla, for dental implant placement. It involves raising the floor of the sinus cavity, which is the air-filled space above the upper teeth, using a crestal approach, meaning the procedure is performed through the natural ridge of the bone.

The crestal approach avoids large incisions or flaps, making it less invasive than other sinus lift techniques like the lateral window approach.
Bone graft material is introduced into the space created by lifting the sinus floor to enhance bone volume and support the implant.
Some crestal lifts utilize gel pressure techniques, where injectable grafts create hydraulic pressure to lift the sinus membrane, reducing surgical time and improving patient comfort.
The crestal approach offers several advantages, including:
It may not be suitable for all patients, particularly those with severe bone loss or an oblique sinus floor.
There is a risk of membrane perforation, although the crestal approach minimizes this risk compared to other techniques.
Rotary instruments used to create a small opening in the bone, allowing for controlled elevation of the sinus membrane.
Specialized instruments that facilitate precise preparation of the site and help prevent membrane perforation.
Using sterile water or saline to gently elevate the sinus membrane after the osteotomy.
Materials like bone graft, xenografts, or alloplastic grafts, are used to fill the space created by the sinus lift.
Once the bone volume is sufficient, the dental implant can be placed.
Overall, the crestal sinus lift is a valuable technique for enhancing bone volume in the upper jaw, allowing for successful dental implant placement in patients with limited bone height
A more common technique where a window is created in the side of the sinus. A lateral window sinus lift, also known as a sinus floor elevation, is a dental procedure that increases bone volume in the upper jaw to prepare it for dental implants. This is achieved by creating a window in the lateral wall of the sinus cavity, lifting the sinus membrane, and placing bone graft material to fill the space.
When bone in the upper jaw is insufficient, it can make dental implant placement difficult or impossible.
This bone loss can occur naturally over time after tooth loss or due to certain medical conditions.
Sinus lifts address this bone deficiency by adding bone where it's needed for implant support.
A window is created on the lateral wall of the maxillary sinus.
The sinus membrane, which lines the sinus cavity, is carefully lifted to expose the sinus floor.
Bone graft material is placed in the space between the lifted membrane and the sinus floor.
The window may be closed with a barrier membrane or the bone window itself, depending on the situation.
The lateral window provides direct access to the sinus cavity, allowing for better placement of the bone graft.
This technique is known for its predictability in bone growth, making it a reliable option for sinus augmentation.
It's suitable for patients with moderate to severe bone loss (less than 6mm of residual bone height).
Maxillary Sinus Lift is small surgical procedure done under local anaesthesia.
An incision is made in the gum to access the maxillary sinus. Access is gained through the gum into the sinus and this allows bone graft material to be placed.
The sinus membrane is carefully lifted, creating space for the bone graft.
The bone graft material is typically taken from the patient's own body (autograft), from a donor (allograft), or from other sources (xenograft or alloplast). It acts as a scaffold for new bone growth. Bone graft material (which can be taken from another part of the body, synthetic, or from another source) is placed in the created space. We use an ultra-purified animal-derived product which originates from bovine (cow) bone. It has been used across the world for over 20 years, on more than 10 million patients and is considered completely safe. It acts as a temporary scaffold allowing your own bone to repair and regenerate itself. The bone graft material is covered by another animal-derived product which is porcine (pig) collagen and this protects the bone graft for the first few weeks while it is healing and is essential for successful regeneration of the missing bone. There are alternatives if you choose not to have animal products, but the scientific literature strongly supports this group of materials as the “gold standard”.
The graft provides the required bone material to allow for implant placement and needs between 5 and 9 months to heal, before the implant can then be placed as a separate procedure.
On some occasions, where there is still some height of bone remaining, implant placement can be completed at the same time as the sinus graft. This has the advantage of only requiring a single operation and usually a shorter healing period of between 4 and 6 months.
If no or very less bone is left during sinus lift procedure, It takes 6 to 9 months to merge bone graft marerial and Once the bone has healed, the implant can be placed.
Closure: The area is closed with sutures.
The bone graft needs time to integrate with the surrounding bone, typically 4 to 12 months, before implant placement.
In some cases, the implant can be placed simultaneously with the sinus lift procedure (one-stage), while in others, the implant is placed at a later stage (two-stage).
Sinus lift surgeries at Magadh Oro Dental Clinic have high success rates, often ranging from 85% to 97%, depending on the technique and individual factors.
