6. Dry socket - Usually after a tooth extraction, a blood clot naturally forms in the socket, the hole in the bone where the tooth has been extracted. It is a surgical procedure that corrects tongue-tie or ankyloglossia by cutting the lingual frenulum. Now we are using soft tissue laser for these type of surgery. Temporo Mandibular (Jaw) Joint - It is the small joint located directly in front of the ear. This joint allows for movement of the lower jaw during opening and closing. TMJ arthrocentesis also known as TMJ washout, is a minimally invasive procedure that can help relieve pain in the temporomandibular joint (TMJ) Dr. Ritesh Vatsa inserts two sterile needles into the joint space and flushes it with a sterile solution under pressure. The fluid can be saline or lactated Ringers solution. The procedure can also involve injecting intra-articular medicines or protein rich plasma from your blood to reduce inflammation. The fluid is then allowed to flow back out, taking with it loose material and inflammatory mediators. The pressure of the fluid can also stretch the joint. Anesthesia - The procedure is usually performed under local anesthesia or IV sedation. Conditions - TMJ arthrocentesis can be used to treat conditions such as chronic TMJ pain associated with osteoarthritis, acute closed lock, inability to open the mouth, and TMJ click associated with pain. Dental cyst and tumors are a specific type of lesion that occur in the jaws because of the tooth that are present or impacted in the lower or upper jaw, or a long standing pathology of the jaw, sometimes even unknown. Some of the dental cyst are also classified as tumors based on their severity and classification. These growths are usually noncancerous (benign), but they can be aggressive and invade the surrounding bone and tissue and may displace teeth.

The Surgical options for jaw tumors and cysts vary, depending on the type of growth or lesion you have, the stage of growth, and your symptoms. Our Oral maxillofacial surgeon Dr. Ritesh Vatsa treat jaw tumor or cyst usually by surgery, however in some cases, by medical therapy or a combination of surgery and medical therapy. Growths in the oral cavity are typically benign in nature or malignant lesion, all suspicious lesions should be biopsied.

This is a relatively common, slow-growing, usually noncancerous (benign) tumor. It develops most often in the jaw near the molars and can invade local structures such as bone and soft tissue. Odontogenic keratocyst.
Also referred to as keratocystic odontogenic tumor because of its tumor like tendency to recur after surgical treatment, this slow-growing, benign cyst can be destructive to local structures. Most often the cyst develops in the lower jaw near the third molars. These cysts may also be found in people with an inherited condition called nevoid basal cell carcinoma syndrome.

In some cases, treatment may be medical therapy or a combination of surgery and medical therapy.

Mandible fractures are treated either closed (maxillomandibular fixation, splinting, modified diet) or open (plates and screws, interosseous wiring, lag screws).The technique chosen depends on a number of variables including, dental status, fracture characteristics (open, closed, favorable, unfavorable, comminution, bone lose, mechanism of injury, contamination or frank infection etc.), fracture location, associated injuries, patient mental status and patient desires. The most important considerations are the dental status, fracture characteristics and fracture location.
Closed techniques involve the relative fixation of mandibular fragments allowing indirect bone healing, which progresses through granulation, fibrosis, cartilage and eventually bone deposition and remodeling. Closed techniques are best used in cases with favorable, closed fractures and in patients with full dentition. Mandible fractures in children heal quite rapidly and because mixed dentition often precludes the placement of screws in the mandible, closed techniques using some form of maxillomandibular fixation are preferred in many pediatric mandible fractures. Condylar neck fractures have traditionally been managed in a closed fashion with a short course of intermaxillary fixation and soft diet. Despite the nonanatomic reduction of the condylar head remodeling occurs over a period of months. Maxillomandibular fixation may be supplemented with interosseous wiring of bone fragments ensuring adequate bone contact and a stable reduction.
Open reduction - There are a number of different techniques used to internally fixate mandible fractures. Simple interosseous wiring to supplement intermaxillary fixation is a form of open reduction and internal fixation. There are a variety of plate and screw systems available for the repair of mandible fractures. Compression plating of mandibles has become popular over the last 10 years. Compression plate fixation allows direct bone healing and in most cases eliminates the need for 4 to 6 weeks of maxillomandibular fixation needed with closed reduction techniques. Compression plates used on the inferior cortex must always be used with some form of tension band as discussed above. A special type of compression plate called an eccentric dynamic compression plate can be used without a tension band. This plate has sliding ramp holes which allow compression of the fracture not only in the area of the plate but also at the tension side of the mandible. In general the application of compression plates requires that the angle of the fracture be no more than 30 degrees from a line perpendicular to the long axis of the plate. If an angle greater than this exists the compression plate will cause the fragments to slide against one another rendering the reduction nonanatomic and not in compression.

12 Simple steps that ensure your recovery from Oral & Maxillofacial Surgery goes smoothly

It normally takes a few days to recover after a tooth extraction. The following steps ensure that your recovery goes smoothly. 1. Apply an ice pack to your cheek directly after the procedure to reduce swelling. Use the ice pack for 10 minutes each time. 2. After the dentist places the gauze pad over the affected area, bite down to reduce bleeding and to aid in clot formation. Leave the gauze on for three to four hours, or until the pad is soaked with blood. 3. Take any medications as prescribed, including over-the-counter painkillers. 4. Rest and relax for the first 24 hours. Do not jump immediately into your regular routine the following day. 5. Don’t use a straw for the first 24 hours. 6. Don’t smoke. 7. Don’t rinse for 24 hours after the tooth extraction, and spit only gently. 8. Use pillows to prop your head up when you lie down. 9. Brush and floss your teeth like normal, but avoid the extraction site. 10. The day after the procedure, eat soft foods, such as yogurt, pudding, and applesauce. 11. After 24 hours, add a half-teaspoon of salt to eight ounces of warm water to rinse out your mouth. 12. As you heal over the next few days, you can slowly reintroduce other foods into your diet. If you are experiencing pain that isn’t going away after several days or signs of an infection, including fever, pain, and pus or drainage from the incision - make an appointment with Dr. Abhishek Kumar as soon as possible.
